As mentioned, it's possible that this is because your server trashes dupes. It's also possible (given that we've established a limit to how -early- the dupes could have been generated in the process [not very], but haven't established a limit to how late they are being generated) that the issue is actually a very low time limit for acknowledgement >from the recieving smtp server.
