Aliera knew that Sethra had failed, but did nothing. The tribe was shamed, Dolivar was tortured and expelled, and Sethra feels really bad about this, especially if we consider that she was consciously acting in a way that would create the tribe of the Jhereg. She possibly even manipulated Dolivar into doing whatever it was that he did, as Quion was manipulated into doing what he was going to do anyway. She knew that he would be tortured and expelled, but for whatever reason, this was necessary. Just because something is necessary doesn't mean that Sethra can't feel badly about the individuals she harms in accomplishing said necessity. Thus, guilt. Or perhaps I'm overthinking it, and she does what's necessary now even as she did what was necessary then, which is to guide and protect Dolivar/Vlad until that which must be accomplished is accomplished. She still might feel a little guilt, and so she tries to watch over Vlad however she can, if it doesn't conflict with her goals.
